HOW DO I...
MAKE AN APPOINTMENT?

Appointments to see the doctors, nurses and healthcare assistants can be made up to six weeks in advance by calling in or telephoning the surgery on 020 8504 0532 or 020 8504 6686. If you cannot keep an appointment, please inform us as soon as possible.

HOW TO GET THE BEST OUT OF THE APPOINTMENT SYSTEM

It is not necessary to call at 8.00am to get an appointment - you can call throughout the day.

If you feel you need to be seen in the next day or two you can call any time during the day and speak to our duty doctor who will decide whether your problem can be dealt with over the phone, if you need to see the duty doctor that day or if you need to see your own doctor in the near future.

For more routine matters, try your best to book an appointment in advance - you can get appointments up to six weeks ahead.

Alternatively, if you have a problem that can be dealt with on the phone please call us for a telephone appointment and your doctor will call you back.

And please remember - an awful lot of things can be carried out directly by our excellent team of nurses, healthcare assistants and receptionists - not to mention our local chemists. For example, queries about referrals, blood test results, blood pressure and other long-term illness reviews.

OBTAIN A HOME VISIT?

Patients are requested to telephone before 10.00am if a visit is required that day. Emergency visits only will be arranged after that time. Please give the receptionist as much information as possible to enable the doctor to allocate priority to home visits.

Please remember that several patients can be seen at the surgery in the time it takes to visit someone at home.

REGISTER AS A NEW PATIENT?

Please bring your NHS medical card and proof of identity such as a utility bill and a current passport. New patients are automatically assigned to a named GP, although their first appointment will depend on availability and the reason for the consultation, eg whether urgent or routine. If registering a baby or young child please bring their 'red book' with you.

NEW PATIENT QUESTIONNAIRES AND MEDICALS

All patients must complete a new patient registration questionnaire. Some new patients aged 15 and over may be also required to have a registration medical with a practice nurse or healthcare assistant although this will not delay their registration with the practice.
